New CHF Patient Guide Launches Across the System

With the goal of keeping patients healthy and out of the hospital, a team of UMMS clinicians, pharmacists, care managers and nurses have built a standard series of care steps and educational tools for treating patients with congestive heart failure (CHF). The CHF Pathway is one of several Epic Clinical Pathway tools being implemented across the System.

Epic Colorectal Surgery Pathway Tracks Milestones Toward Patient Health

A new clinical pathway implemented across the System has standardized the plan of care for the more than 1,300 patients undergoing colorectal surgery at UMMS hospitals each year. The tool is the first of many pathways to be developed as part of the System’s commitment to encouraging innovation and the implementation of effective and sustainable models of care that lead to enhanced quality of care for our patients.
